Redefining exercise prescription in chronic pain.
FIBROGENHIIT is not merely a clinical exercise study.
It is a precision “Exercise as Medicine” research platform integrating exercise physiology, autonomic regulation, genetics, and environmental modulation to personalize therapeutic strategies in fibromyalgia.
Developed at the University of Cagliari (Department of Medical Sciences and Public Health), the project challenges the traditional “one-size-fits-all” paradigm of exercise prescription by advancing a mechanism-based, biologically informed, and data-driven model of intervention.
By combining randomized clinical design, physiological phenotyping, autonomic biomarkers, and gene × environment interaction analysis, FIBROGENHIIT establishes a scalable framework for precision non-pharmacological medicine.
We invite leading international academic institutions, clinical centers, and research laboratories to join this expanding global network and contribute to the development of a multicenter precision exercise platform positioned for competitive international funding and long-term scientific impact.
International Collaboration Opportunity for a Precision Exercise Medicine Consortium
FIBROGENHIIT does not seek isolated collaborations.
We are building the foundations of a global consortium in Precision Exercise Medicine for chronic pain.
We are therefore looking for institutions with the capacity, vision, and infrastructure to co-develop large-scale international research initiatives.
FIBROGENHIIT is conceived as an international, consortium-ready platform designed to support:
-
Multicenter randomized controlled trials
-
Gene × Environment interaction modeling
-
Mechanism-based exercise prescription
-
Integration of digital monitoring and AI-driven prediction
-
Development of precision lifestyle medicine frameworks
The platform is positioned for competitive participation in major international funding frameworks, including Horizon Europe and other global research programs.
Universities and clinical centers.
We are seeking universities with a strong track record in International competitive funding, as well as academic clinical centers with proven expertise in chronic pain research and care.
Research laboratories.
We invite research laboratories with complementary scientific expertise in the fields of physiology, genetics, biomedicine, and pain research.
Advanced data infrastructures.
Istituzioni con infrastrutture dati avanzate ed expertise in analisi di big data e intelligenza artificiale per modellazione predittiva.
What Makes FIBROGENHIIT Unique
FIBROGENHIIT is not simply another exercise intervention study in fibromyalgia.
It is a precision “Exercise as Medicine” research platform designed to understand for whom exercise works, under which conditions, and through which mechanisms—and to scale this approach worldwide.
Precision beyond the average.
While most studies report average clinical improvements, FIBROGENHIIT focuses on response variability, identifying clinically meaningful responders and characterizing the determinants of inter-individual differences in outcomes.
This allows us to move beyond the simplistic notion that “exercise works” and toward a mechanistic understanding of for whom, under which conditions, and why it works.
Importantly, the intervention is not generically prescribed. The protocol has been carefully designed within a structured methodological framework, and exercise intensity is individualized through physiological profiling. When available, cardiopulmonary exercise testing (CPET) is used to define peak heart rate and ventilatory thresholds. In settings where CPET is not feasible, validated alternative approaches are applied, including heart rate reserve (HRR), field-test–derived maximal heart rate, and standardized submaximal performance assessments.
Training load progression and internal response are continuously monitored through heart rate dynamics, perceived exertion, and symptom tracking. These multidimensional data streams are integrated within a data-driven framework, where AI-supported analytical models contribute to adaptive intensity calibration and longitudinal response profiling, reinforcing a precision-based intervention strategy.
An integrated, mechanism-based model.
FIBROGENHIIT uniquely integrates, within a standardized framework:
-
A randomized controlled clinical design
-
Individualized HIIT prescription based on CPET (precision dosing), heart rate reserve (HRR), and 6-minute walk test (6MWT)–derived parameters
-
Autonomic regulation monitoring (HRV – RMSSD) as a mechanistic biomarker
-
Targeted genetic analysis to explore biological modulators of exercise response
-
Indoor versus outdoor (“Green HIIT”) comparison to assess environmental modulation
From Strategic Interest to Institutional Partnership.
FIBROGENHIIT is structured as a coordinated international research platform. To ensure methodological integrity, scientific coherence, and long-term consortium development, access to the intervention protocol follows a formal engagement process.
FIBROGENHIIT is not an open-distribution initiative. It is a strategically curated international research network built through selected institutional partnerships.
Step 1: Institutional Expression of Interest.
Submit your formal expression of interest to initiate a dialogue regarding potential collaboration with FIBROGENHIIT.
Step 2: Strategic Alignment Meeting.
A dedicated meeting to align strategic objectives and assess institutional synergy within the FIBROGENHIIT consortium framework.
Step 3: Controlled Access to the Protocol.
Following strategic alignment, controlled access will be granted to the FIBROGENHIIT implementation framework and research protocols.
Institutions joining at this stage become founding contributors to a global Precision Exercise Medicine platform, strategically positioned for competitive international funding, multicenter expansion, and long-term scientific leadership.
